Pulsus paradoxus-
Pulsus paradoxus is defined as an inspiratory systolic fall in arterial pressure of 10mmHg. It can occur in cardiac tamponade,  massive PE, severe COPD and hypotension/shock. It should be kept in mind during cardiac catheterisation procedures like ASD device closure. Sometimes while doing atrial perforation, cardiac free wall may get perforated. Recognition of pulsus paradoxus will help in identiying tamponade and manage the case.
